Within our "55 and Older Memory Care Communities," it is our goal for each resident to preserve dignity, embrace independence, and exercise freedom of choice. Our team is trained to value each resident and recognize different likes, dislikes, and needs. We know memory loss affects everyone differently and therefore, we engage our residents on an individual basis in an effort to provide the best possible results.
Memory Care Communities at Summerfield offer unique services such as the following:
We compile history of resident from childhood to the present.
We highlight special yearly dates in a resident’s life such as birthday, anniversary, holidays, etc., and reinforce that in their daily lives.
We work with family members to gather additional information related to the resident's well-being.
We consult with the resident’s physician in developing an in-house plan of care that will provide monitoring of resident's actions through various programs and activities.
We maintain a daily log of dietary and hydration consumption
Individualized Plan of Service
Our staff, nurses and caregivers work together as a team in determining an Individualized Plan of Service for residents’ well-being. We know it’s not just about remembering how to perform daily activities –it’s helping them recall why their entire life is worth remembering! Our staff involves the family as well. We have a team approach to our care.
An individualized service plan might include the following:
Bathing, showering, and toileting
Dressing, grooming, and personal hygiene
Reminders and escorts to dining meals
Continence management
Mobility and transferring assistance
Socialization and daily activities
